If you are enrolled in eDocuments, your 5498-SA form will be available in Blaze Digital Banking by May 31. If you are not enrolled in eDocuments, they will be mailed to you.
The 5498-SA is issued to report contributions, rollover contributions, and the fair market value (FMV) of your HSA, Archer MSA, or Medicare Advantage MSA account.
